This research paper evaluates the HeartConnect model of care, which aims for greater collaboration between general practitioners and cardiologists.

Muscle matters: A proposed algorithm to guide screening, assessment and management of poor muscle health in primary care

The aim is to develop a proposed clinical muscle health monitoring and management algorithm for use by healthcare professionals to identify patients with or at-risk for poor muscle health.
