1800PREVENT: Your partner in type 2 diabetes prevention
SPONSORED: Preventing type 2 diabetes is a team effort, with trusted resources and support available to patients between consults.
This service is designed to complement GPs’ clinical care by helping patients stay on track between appointments.
1800PREVENT is a free, easy-to-access service designed to support the care GPs already provide for patients at risk of developing type 2 diabetes.
We’re helping patients understand that risk is about more than just food and movement – age, family history, and cultural background all play a role.
Who will benefit from the service?
This service is ideal for patients who want to understand their risk of type 2 diabetes, learn practical steps to prevent it, or access free support offered locally throughout Australia.
What services are offered to patients?
What the service offers:
- Risk assessment support: Personalised guidance on using tools like the Australian Type 2 Diabetes Risk Calculator to understand the five-year risk of developing type 2 diabetes
- Health education: Clear, practical information to help patients understand type 2 diabetes and take steps to reduce their risk
- Local referrals: Direct connection to free, state-specific diabetes prevention support services across Australia
- Helpful resources: Access to practical guides and materials that support lifestyle changes and long-term wellbeing, reducing patients’ risk of type 2 diabetes
How does it work?
This service is designed to complement GPs’ clinical care by helping patients stay on track between appointments.
Through a simple and structured approach, the service supports early intervention and long-term lifestyle change in three simple steps:
- Talk: Start the conversation and understand the risk. Empower patients to take the first step. With up to 58% of cases prevented or delayed, there's real power in acting early. A brief, free chat helps patients assess their risk and opens the door to ongoing lifestyle support
- Act: Make informed choices early and take action. We help your patients understand their options and act early because type 2 diabetes isn’t just about food and movement. Age, family history, and ethnicity all play a role. Our team will help patients understand their risk and guide them towards the right next steps
- Prevent: Supporting small steps today for better health tomorrow. With the right support, patients get personalised guidance on nutrition, movement, and wellbeing that's designed to help them make lasting, positive changes that fit their life
