GP Aged Care Network is seeking General Practitioners to join a growing RACF visiting service across various areas in South East Queensland. This role offers a supported independent practice model for GPs who want to deliver high-quality, patient-centred care in Residential Aged Care Facilities, without the administrative load and overheads of traditional clinic-based practice.
Key Features
- Retain 80–90% of billings, depending on engagement model

Clinical Scope
- Comprehensive aged care management
- Chronic disease and complex care
- Medication reviews and deprescribing
- Palliative and end-of-life care
- Case conferencing and care planning
- Multidisciplinary care coordination
Support Provided
- Centralised billing and MBS claiming support
- GPACI tracking and support
- Nurse Practitioner and Practice Nurse support
- Administrative and triage support
- IT systems and RACF remote System access
- Scheduling and RACF coordination
- Clinical governance and peer support
