General Practitioner — Telehealth Case Conferencing

Veteran Pathways & Proactive Care is seeking experienced GPs for a fully remote telehealth role delivering structured case conferencing and chronic disease management for veterans and patients with complex health needs.
You will provide clinical leadership within integrated Chronic Care Teams, collaborating with allied health professionals and specialists to deliver coordinated care plans across Medicare and DVA billing streams.
The Role:
Lead telehealth case conferences within multidisciplinary Chronic Care Teams
Provide medical oversight for complex chronic conditions
Undertake CDM reviews, GP Management Plans and Team Care Arrangements
Manage prescribing, referrals and clinical escalation pathways
Approx. 22 patient reviews per 8-hour session via telehealth
Remuneration:
First 3 months: $240/hr (guaranteed)
Ongoing: 70% of billings
Sign-on bonus and anniversary incentive available
Requirements:
MBBS or equivalent, current AHPRA registration
RACGP or ACRRM fellowship (VR GP), or 19AB exemption in DPA locations
Experience in chronic disease management and multidisciplinary care
