About the Role:
- • Conduct scheduled visits to aged care homes and/or in-home aged care patients
- • Build long-term relationships with patients and families
- • Focus on proactive, preventative, and palliative care
- • Work autonomously with the support of our central admin and care coordination team
- • Choose your days and hours — part-time or full-time available

What You Need:
- • FRACGP or equivalent
- • Current AHPRA registration and full Australian work rights
