Sydney Local Health District (SLHD) is seeking an experienced Staff Specialist (General Practitioner) to join our team at Croydon Health Centre.
We are looking for a Staff Specialist (General Practitioner) for the Specialist Team for Intellectual Disability Sydney (STrIDeS), Sydney Local Health District. The primary purpose of the role is to bring specialist general practice expertise to the development and implementation of the Intellectual Disability Shared Care Model that is a component of the ADHD/Intellectual Disability Shared Care Model. STrIDeS/ Department of Community Paediatrics are partnering with Central and Eastern Sydney Primary Health Network (CESPHN).
People with intellectual disability have poorer health. There
has been significant policy and funding enhancement at both state and national
level over the last 8 years or so. Some examples include the Primary Care
Enhancement Programme and National Roadmap for Improving the Health of People
with Intellectual Disability. CESPHN is one of only 4 national pilot sites for
PCEP.
The
ID Shared Care Model has been proposed by CESPHN as an additional
service/system response to the unmet needs of people with intellectual
disability. Given their role as a PCEP site, participation in national network
of PCEPs and extensive connection with SLHD, state and national government and
other ID organisations, such as NSW Council For Intellectual Disability we consider that this may be a solution to the persisting health
inequities experienced by people with intellectual disability.
